What you are describing is exactly what I would expect of this macro (GarminCsvPoiExport-v2.gsk - this forum thread is the support thread for that macro). Note that this macro is from a time when GPSrs did not carry as much information as they do today. So if you are seeking to use capabilities of a relative new GPSr model, then this is probably not the macro you want to use. THe main reason why someone would use this macro with a newer GPSr is that it can carry a nearly unlimited number of waypoints via the POIs but at the price of not showing much other information.
The items that appear to be duplicates are not duplicates; they are the pieces which together are needed to show all the tidbits of info. Generally the nuvi from that era could only display 256 characters in each POI's info box. So longer hints are split up over multiple ones. Doesn't all this information appear when you click the HELP button?
